Letrozole tablets once a week restore fertility in obese men with endocrine infertility, according to a new study from Canada. Use of this new treatment for male infertility contributed to the birth of two full-term, healthy children. The results are presented at the 97th annual meeting of the Endocrine Society in San Diego. “To our knowledge, this is the first successful study using low-dose letrozole in men,” says lead study author Lena Salgado, M.D., an endocrinologist at the Centre Hospitalier de l’Université de Montréal (CHUM).

Letrozole is approved by the FDA (U.S. Food and Drug Administration) to treat HER2-positive breast cancer in postmenopausal women and is also used “off-label” in infertile women to induce ovulation. Recently published studies have shown that obese men are characterized by low testosterone levels, and letrozole normalizes it.

Obesity leads to a hormonal imbalance in men and as a result too much estrogen is produced. The adrenal enzyme aromatase – which is most prevalent in fat – converts the male sex hormone testosterone to estrogen. Letrozole inhibits the action of aromatase and prevents testosterone from being broken down into estrogen.  A team of scientists analyzed data from 12 men with obesity-induced hypogonadotropic hypogonadism who sought medical help for infertility.

Hypogonadotropic hypogonadism is a disease in which there is a decrease in the production of hondotropic hormones that regulate androgen synthesis, particularly testosterone. Low sperm counts can also lead to this disease. In the study, the men were infertile for three years. In the experiment, they received a 2.5 mg tablet of letrozole once a week. Results were evaluated between 2 and 21 months.

According to the results of the study, one man had severe headaches and was replaced with another aromatase inhibitor, anastrozole. Another man did not respond to letrozole treatment because he had comorbidities, including uncontrolled diabetes, which also affects testosterone levels and/or semen quality. In the remaining 11 patients, testosterone levels increased to normal. Estradiol, a type of estrogen, decreased significantly in most men.  Four pregnancies followed: two pregnancies ended in successful delivery; one turned out to be ectopic; another one ended in miscarriage.

Treatment with letrozole is much cheaper and easier than the usual hormonal injections of chorionic gonadotropin (hCG).  The dose of hormone injections needed to obtain normal testosterone levels and sperm production is proportional to body weight, so in obese men the cost of hormone treatment is very high.  Letrozole is a promising drug for the treatment of male infertility with obesity-related hypogonadism.

Because testosterone and anabolic steroids aromatize into estrogens, gynecomastia (breast enlargement) can occur in men and gonadotropin and testosterone levels can decrease. Aromatase inhibitors cannot completely normalize estrogen levels in men. The most serious side effect of aromatase inhibitors is the effect on bone health. Bone mineral density may decrease, leading to an increased risk of fractures. Estrogen is an important hormone not only for women, but also for the bone health of men. The following side effects have been observed with medical use: headache, fever, nausea, eczema, arthralgia, ossification of joints, arthritis, and asthenia. In the treatment of male infertility, side effects were rarely observed.
